Referrals
Referrals ARE NOT required to make an appointment with our Physicians.
However,
if you have medical insurance coverage that requires a prior authorization
for medical appointments with a SPECIALIST, you will need to work
with your Primary Care Physician in obtaining referral authorization
prior to scheduling an appointment.
If
you have seen your Primary Care Physician for the medical problem
you wish to be seen for in our office, it is helpful for us to know
that at the time an appointment is made.
Likewise,
if you have had previous diagnostic tests (x-rays, MRI, CT) completed,
that, too, is helpful in knowing prior to your appointment with
our Physicians.
Financial
Policy
Cascade Orthopedics & Sports Medicine Center, P.C. participates
with Medicare and most commercial and local managed care insurance
plans.
For
workers compensation patients, we need to confirm claim status prior
to appointment.
Co-payments
are required at time of visit. For non-insured patients we ask for
a deposit at time of visit.
Our
Financial Policy will be disclosed at your first visit. If you have
any financial concerns please ask to speak with our Business Office
staff.
Prescription
Refills
It is our office policy not to fill prescriptions during evenings
and weekends. We do not believe it is good patient care to fill
a prescription if the provider does not know the patient or have
the medical record at the time of the call.
Prescription
requests should be addressed during your visits or phoned to the
office during weekday office hours, 8:00 am - 5:00 pm. If you have
a REFILL request, phone your pharmacy and they will forward request
via FAX to our office.
Prescriptions
received after 3:00 pm may not be processed until the next business
day. We suggest you allow one business day for refills to be completed.
